Patients progressing after Lu-177 DOTATATE have limited options. 225Ac-DOTA-LM3 addresses multiple resistance mechanisms: high-LET alpha particles cause complex, harder-to-repair DNA damage (vs more readily repaired beta damage), and antagonist binding accesses more receptor sites. A retrospective series of 35 patients reported acceptable safety and promising survival after progression on Lu-177 PRRT.
